Applying for a Primary Care Provider at the Westshore CHC
How to Apply
If you would like to be considered as a patient at the Westshore Community Health Centre, please complete the following two steps:
Step 1: Register with the Health Connect Registry
Register with the Health Connect Registry. The Health Connect Registry provides a pathway to be connected with a primary care provider in the community. If you are already registered, you do not need to register again.
Step 2: Complete the Westshore Community Health Centre Application
Please complete the application below ensuring that all required and relevant information is included.
The Westshore Community Health Centre serves people living in Langford/Highlands, Colwood, and Metchosin who do not have a regular primary care provider and are experiencing significant barriers to accessing health care including barriers related to income, housing or employment instability, discrimination, limited social supports, health or social complexity, or other social determinants of health.
Applications are reviewed based on this mandate and our available primary care capacity. Applicants who meet the clinic’s mandate will be contacted as space becomes available.
Submitting an application does not guarantee attachment to a family doctor or nurse practitioner.
Please note: We are not a walk-in clinic. Medical services are available to patients who have been attached to a Westshore Community Health Centre primary care provider.
