Social Prescribing – Community Connector (Seniors Services 65+)
The Social Prescribing – Community Connector program helps older adults stay healthy, independent, and connected to their community. Through personalized one-on-one support, the program links seniors to local services, programs, and social supports that improve overall well-being.
Social prescribing is rooted in the understanding that health is more than just medical care. A sense of purpose, social connection, and access to local resources can all play a powerful role in reducing isolation, preventing decline, and improving quality of life.
Our Community Connector works with older adults to identify their goals, needs, and interests. Together, they co-create a wellness plan and access appropriate services and community connections.
Services May Include:
- Access to social and recreational programs
- Housing, food, and income supports
- Transportation referrals
- Friendly visiting or peer mentoring
- Help navigating services and filling out forms
- Cultural and spiritual connection opportunities
- Volunteer and intergenerational engagement
Who Is It For?
- Adults aged 65+ living in Langford, Colwood, Metchosin, or Sooke
- Seniors who may be isolated, facing a health or life transition, or needing additional support
For Family Members & Caregivers:
If you’re supporting an aging parent or loved one, navigating services can be overwhelming. The Community Connector can help simplify the process — offering guidance, referrals, and hands-on support to ensure your loved one stays connected, engaged, and well-supported close to home.
How to Access This Program:
- Referrals are accepted from health professionals, community agencies, or family members.
- Seniors can also self-refer.
